Whether you’re a county case manager, provider, or family member, this page shows the quickest way to refer someone to TSE for employment or day services.

Job support, career development, and ongoing employment assistance.
Center-Based / Day Services
Pre-vocational supports and day programming.
How it Works
1 — Referral
Case manager submits required documentation so TSE can evaluate needs and interest.
2 — Introductory Visit
Meet TSE staff. If site-based services are likely, a site tour can be scheduled.
3 — Intake & Start
Services confirmed, orientation given, start date set and a 45-day follow-up scheduled.
- Required Documents:
- Case manager letter identifying requested services
- Current Community Support Plan (CSP) or Coordinated Services and Support Plan (CSSP)
- Most recent Progress Report or CSSP Addendum from current/previous provider
- Current psychological evaluation
- Current physical examination
- Work history or other skill-related information

Transportation Support — Bus Cards & Metro Mobility
TSE administers Metro Transit bus cards and Metro Mobility cards for individuals receiving Medicaid (MA) waiver services. To refer a client for a bus card, follow the steps below.
- Contact the TSE main office to check availability: 651-489-2595 ext. 0 or metro@tse-inc.org (this email is for bus card referrals only).
- Once TSE confirms capacity, complete the Bus Card Referral Form linked below.
